To be eligible for compensation, individuals must have received a data breach notice from PracticeMax in June 2022, indicating potential compromise of their information in a cyberattack from early 2021. They should not have already been reimbursed through credit monitoring or identity theft insurance and must have documented losses between May 1, 2021, and October 5, 2023. Claimants must also demonstrate reasonable efforts to seek reimbursement for their losses.

In addition to monetary compensation, qualifying claimants can receive two years of free credit monitoring and identity theft protection services, including single-bureau credit monitoring and $1 million in identity theft insurance.

Residents seeking to claim compensation must submit a valid claim form by the deadline, providing necessary personal information and supporting documents such as receipts or bank statements. Failure to meet the submission deadline may result in forfeiture of potential compensation.

Beyond the PracticeMax settlement, eligible residents of Rochester, New York, may also benefit from a guaranteed basic income program initiated by the city. Under this program, eligible residents can receive monthly payments of up to $500, funded by $2.2 million from the city’s American Rescue Plan Act allocation. To qualify, individuals must have resided in Rochester for at least one year, be at least 18 years old, and have an income at or below 185% of the federal poverty level.

Residents are encouraged to take advantage of these opportunities for financial support and to stay informed about developments in payment programs and initiatives within their communities.
